Why is Care Coordination Important?

Families face many barriers when trying to access and navigate complex systems of care. These barriers include:

  • Lack of knowledge about what resources are available
  • High cost of care
  • Lack of availability of services, such as waiting lists
  • Feelings of hopelessness, overwhelmed and anxiety due to the family’s situation

In most cases, successful linkage to resources requires knowledge of programs and eligibility requirements and persistence in overcoming barriers. The Care Coordinators’ role is to make those important linkages for families happen.

When you are looking for specialized services for a child or a young adult and don’t know where to turn, 211 is here to support your family.

When you call our Helpline, please ask to speak to a Special Needs Care Coordinator.

They will assess your needs by asking critical questions to seek available services and explore the most appropriate community referrals.

With your consent, the Care Coordinator will connect you to the right service and advocate on your behalf by sharing the information with your preferred community providers. To ensure you are connected to the help you need and provide additional support, a Coordinator will offer you a follow-up.

What To Expect

What is the Special Needs Connections Case Management Program?

Do you need extra support to link with services, find therapies, obtain benefits, seeking respite care, need in-home supports, etc.? The Special Needs Connections Case Management Program is here to support your family.

In collaboration with Jewish Adoption and Foster Care Options (JAFCO), the Special Needs Case Management program offers intensive supportive services, such as coordinated case management and face-to-face support for families and caregivers of youth with disabilities.

The purpose of this program is to empower families so they can meet their goals and their children can thrive while overcoming barriers to accessing services.

The distinction between care coordinator and case management is care coordination is telephone support and short-term care. While the case management program care is intensive, face-to-face, and generally lasts up to 3-6 months.

Who is Eligible for Special Needs Connections Case Management?

Children birth through 22 years of age with special needs who have a physical, developmental, sensory, or learning disability. The disabilities served by the program are:
  • Austims
  • Developmental delays
  • Speech or language impairments
  • Intellectual disabilities
  • Down syndrome
  • Deaf or hard of hearing
  • Traumatic brain injury
  • Visual impairments
  • Spina bifida
  • Cerebral palsy
  • Other health impairments that need special education and related services.
