Community Connections Council

Community Connections Council (C³), powered by Unite Us, is a progressive movement that harnesses the value of cross-sector collaboration and data sharing to efficiently address the social drivers of health from a closed-loop, person-centered, and whole-person approach to best provide customized, high-quality care.  Systemic improvements will be made through increased levels of transparency and accountability.  Ultimately, payments for social care will be on par with healthcare and transacted timely through integration with . Imagine procuring, delivering, and reimbursing for social care in one seamless C³ process.  211 Broward is proud to begin C³ pilots that are community-driven and that seek to demonstrate the return on investment (ROI #1) and rate of impact (ROI#2) that are possible with scale.  Think of C³ as the connective tissue in a trusted network of care.​​

Project Partners

C3 Concludes Recent Project: Medicaid Redetermination Project (MRP)

C3 and 211 Broward share the successful completion of the MRP Project. This 2024 initiative reports a 93% successful outcome in connecting individuals in Broward to needed access to health insurance. C 3 exceeded the other 5 states funded in this 2024. The achievement is attributed to C 3 ’s strong partnerships, 211’s technology and in-reach efforts, as well as the C 3 Platform.

More about the Medicaid Redetermination Project: The Infant-Toddler Court Program National Resource Center partners with Coast to Coast Legal Aid, the Urban League of Broward County, and 211 Broward (along with other community agencies) provided access to services and supports to individuals and families related to Medicaid eligibility redetermination and enrollment for Broward County Residents. This program assisted over 80 families and individuals in accessing their health insurance needs.
