Youth Care Coordination and Case Management

Start Treatment Early
Being a young person at times can be challenging. Young people can experience mental illness and substance misuse. Adolescents are affected by mental health needs, including anxiety, disruptive behavior, and mood changes.

 

Some of these issues can result from drug or alcohol use, which can lead to life-threatening situations. Youth struggling with mental health disorders or substance use can improve their quality of life with the proper support and resources.

​Children’s mental health is integral to their overall health. The earlier treatment is started for a young person with symptoms of a mental illness and substance use concerns, the more effective it can be. Early treatment can help prevent more severe, lasting problems as a child grows up.
 
Learning the Warning Signs
It can be tough to tell if troubling behavior in a child is just part of growing up or a problem that should be discussed with a health professional. Treatment for youth with mental illness and substance use is effective. Contact a health professional if:
Warning Signs:
 
  • Have frequent tantrums or are intensely irritable much of the time
  • Often talk about fears or worries
  • Complain about frequent stomachaches or headaches with no known medical cause
  • Sleep too much or too little, have frequent nightmares or seem sleepy during the day
  • Are spending more and more time alone and are not interested in playing with other children or have difficulty making friends
  • Struggle academically or have experienced a recent decline in grades
  • Repeat actions or check things many times out of fear that something bad may happen
  • Have lost interest in things that they used to enjoy
  • Have low energy
  • Diet or exercise excessively
  • Engage in self-harm behaviors (e.g., cutting or burning their skin)
  • Smoke, drink alcohol
  • Engage in risky or destructive behavior alone or with friends
  • Have thoughts of suicide
  • Have periods of highly elevated energy and activity, and require much less sleep than usual…

Our Behavioral Health INFOline is a free program that provides children, young adults (ages 21 and younger) and their families with care coordination, linkage to specialized mental health services, case management, and substance use programs.

Care Coordination

What is the Behavioral Health Case Management Program?

Do you need support to link with services, find therapies, substance use programs, family support, etc.? The Behavioral Health Case Management Program is here to support your family.

In collaboration with Broward Behaviroal Health Coaltion (BBHC), the Behavioral Health Case Management program offers intensive supportive services, such as coordinated case management and face-to-face support for families and caregivers of youth with mental health and substance use concerns.

The purpose of this program is to empower families so they can meet their goals and their children can thrive while overcoming barriers to accessing services.

The distinction between care coordinator and case management is that care coordination is telephone support and short-term care. While the case management program care is intensive, face-to-face, and generally lasts up to 3 months.

Who is Eligible for the Behavioral Health Case Management Program?

 

Children and young adults 21 years old or younger with behavioral health concerns. Please know the youth or young adult does not need a mental health diagnosis.
