Why is Caregiver Navigation important?
Caregivers face many barriers when trying to access and navigate complex systems of care. Successful linkage requires knowledge of programs, eligibility requirements, and persistence in overcoming barriers. The Caregiver Navigator’s role is to create those essential linkages.
Offers a range of supports to caregivers (18+) who are taking care of a loved one (18+) to help them navigate their roles as family caregivers. The services include:
- Care coordination
- Education
- Specialized assessment, information, and referral for caregiving
- Advocacy
- Crisis support
- Developing a caregiving plan
- Offering supportive follow-ups
- Supporting life balance

What happens when a caregiver reaches 211?
211 is your First Call for Help!
When you are looking for specialized services for a loved one and don’t know where to turn, 211 is here to support you.
When you call our Helpline, please ask for the Caregiver Navigation program.
They will assess your needs by asking critical questions to seek available services and explore the most appropriate community referrals.
With your consent, the Caregiver Navigator will connect you to the right service and advocate on your behalf by sharing the information with your preferred community providers. A coordinator will offer you a follow-up to ensure you are connected to the help you need and provide additional support.
